Absence Notification Form

Absence Notification Form Link

When a child returns to school after an absence, a parent or legal guardian must either send a written note to the teacher explaining the reason for the absence or complete the electronic absence form.

For absences of one or two days, a note from the parent or legal guardian must be submitted to the teacher within two days of the student’s return. The note should include the dates and reason for the absence, along with a daytime phone number where the parent can be reached. Alternatively, the electronic form may be completed.

For absences of three or more days, a doctor’s note is required.

Teachers will review any missed work with students returning from an excused, lawful absence. Parents are encouraged to contact the teacher directly to request missed assignments.

Absences not covered under the school’s attendance policy or those not accompanied by an acceptable note within 48 hours will be marked unexcused. In such cases, students may not be permitted to make up missed work.

If a student will be absent for more than three days due to medical reasons, parents should inform the school in advance so arrangements for make-up work can be made.
